New patients are requested to print and fill-in the Demographic Information form as well as the Intake History form and bring them to their first clinic appointment. Simply click on the appropriate tab to access the form(s) needed for your first clinic appointment, or to obtain answers to questions you may have regarding insurance, billing, credit and other topics.
